Before you give up on e-bikes due to respiratory illness, I would recommend you look at class 3 models that can be operated throttle-only up to 20mph. I believe they are legal where other bikes/e-bikes are in most of the US.
More expensive than standing scooters (on average), but far more practical and versatile. In addition, some of these have adopted the form factors of other traditional motorized two wheeled vehicles such as seated scooters, mopeds, mini-bikes, and even dirt bikes.
