Of the options listed, I concur that this is the best. msword is not portable enough. Personally, I think the best thing to do would be to create an online form, then point them to the form. Of course this might be cost/time prohibitive, but would provide the best experience and quickest solution for those who opt to fill out the questionnaire. You could write your own forms using this:
