Loud shreaking sound on startup

Let's just say someone who lives near me has a car that does this and sound like it has an exhaust leak as well. They just keep driving the car like that and don't bother getting it fixed. Curious as to what would do that, maybe a bad torque converter?

More likely coming from air or low fluid in power steering system or a belt of some sort. Excess vacuum causes a squeal but usually not heard over engine/exhaust noise.

They seem real interested in their car.
